I used to have contact problems with my CST due to the construction of the contact button on the shoe.
I posted about this in this forum a couple times.
I was thrilled when I saw the new CST2 and noticed that they fixed the problem with the shoe contact!
And as a bonus, they changed the battery to a much more common type...
GREAT I thought! I bought 3 to replace my CSTs...
Well, I'm really not sure what's causing it, but the CST2s misfire quite often, maybe one out of 10-20 trys. It's not a contact issue. Just trying again almost always fires the E640. Also, when I'm using a multiple light setup, it seems that different lights misfire. I'm a computer engineer, and my assessment of the problem is that the digital trigger codes being sent out aren't being transmitted cleanly and sometimes different bits are missed... I'll be interested to see what Tech Support has to say...
All three of my CST2s do this, it's not simply one defective unit, it's a design issue of some kind.

Have you tried changing frequencies? This sounds a lot like radio interference, as it manifests itself among all three transmitters similarly, and does not affect all lights simultaneously. If the transmitter failed to fire, then the lights would simultaneously fail to fire.
